NAD+ vs. Niagen: What’s the Difference— and Which One Might Be Right for You?

If you’ve been exploring IV therapy, longevity, energy, or cellular wellness, you’ve probably heard a lot about NAD+. More recently, you may have also come across Niagen® (nicotinamide riboside) and wondered: Are they the same thing? Is one better than the other?


While NAD+ and Niagen are closely related, they aren’t identical—and understanding the difference can help you make a more informed decision about your wellness routine.


At The IV Nurse, we offer both NAD+ and Niagen through IV infusion and intramuscular (IM) injection, allowing treatment to be individualized based on your goals, preferences, and tolerance.


First, What Is NAD+?

NAD+, short for nicotinamide adenine dinucleotide, is a coenzyme naturally found in virtually every cell in the body. It plays an essential role in several fundamental cellular processes, including:

·       Participating in DNA repair processes

So, What Is Niagen?

Niagen® is a patented form of nicotinamide riboside (NR), a vitamin B3-related compound that acts as a precursor to NAD+. That distinction is important.


With NAD+ therapy, NAD+ itself is administered. With Niagen, the body receives NR, which cells can use as a building block to produce NAD+ through the body’s natural metabolic pathways.


A simple way to think about it is:

NAD+ = the finished molecule

Niagen (NR) = a building block your body can use to make NAD+


Clinical research on nicotinamide riboside has demonstrated that NR supplementation can increase NAD+ levels in humans. Researchers continue to investigate what those increases may mean for healthy aging, metabolic health, cardiovascular health, muscle function, exercise, and other aspects of wellness.



NAD+ vs. Niagen: How Do They Compare?

Although both approaches are connected to NAD+ biology, they take different routes.


NAD+ Therapy

NAD+ therapy provides NAD+ directly rather than supplying a precursor that first needs to be converted.


People commonly seek NAD+ therapy as part of wellness programs focused on areas such as energy, mental clarity, recovery, healthy aging, and overall cellular wellness.


One important consideration is tolerability. NAD+ IV infusions are typically administered slowly. Some people experience temporary sensations such as nausea, abdominal discomfort, flushing, headache, chest pressure, or an overall uncomfortable feeling when NAD+ is infused too quickly.


Adjusting the infusion rate can often make the experience more comfortable.


Niagen / Nicotinamide Riboside

Niagen takes a different approach by supplying nicotinamide riboside, which the body can convert into NAD+.


One of NR’s strengths is the amount of research demonstrating its ability to raise NAD+ levels in humans. Niagen has been evaluated in more than 45 published human clinical studies, and its broader research program includes hundreds of collaborations with researchers and institutions around the world. Importantly, the vast majority of studies conducted through the Niagen Research Program are reported to be investigator-initiated and third-party funded.


For clients interested in supporting their body’s NAD+ production, Niagen may provide an appealing alternative to administering NAD+ itself.


What Does the Research Actually Say?

This is where it’s important to separate the excitement surrounding NAD+ from what has actually been established in human clinical research.


Scientists have accumulated substantial evidence showing that NAD+ is critically important to cellular metabolism and that NAD+ biology changes with aging. Increasing NAD+ availability has also produced intriguing results in laboratory and animal research.


Human research, however, is still developing.


Nicotinamide riboside has relatively strong human evidence showing that it can increase NAD+ or related NAD metabolites. What is less established is whether raising NAD+ consistently translates into major improvements in outcomes such as energy, cognition, athletic performance, weight loss, or lifespan in otherwise healthy people.


Research into directly administered NAD+ is also growing, but there is currently less robust clinical evidence for IV or IM NAD+ therapy than the popularity of these treatments might suggest.


That doesn’t mean NAD+ therapy has no potential benefit. It means we believe clients deserve to understand the difference between biological rationale, emerging research, individual experience, and proven clinical outcomes.


Why Is There More Research Behind Niagen?

Part of the difference may come down to research investment and intellectual property.


Niagen is a patented form of nicotinamide riboside. Patent protection can create commercial incentive to invest in standardized manufacturing, product development, regulatory work, clinical trials, and continued research.


NAD+, on the other hand, is a molecule that occurs naturally in the human body. A naturally occurring molecule itself generally cannot simply be patented as a proprietary invention in its natural form. Because no single company can exclusively own the naturally occurring NAD+ molecule itself, the commercial incentives surrounding direct NAD+ research are different.


Is Niagen Better Than NAD+?

Not necessarily. Rather than thinking of one as universally “better,” it may be more useful to think about how each supports the NAD+ pathway.


NAD+ therapy delivers NAD+ directly. Niagen provides nicotinamide riboside, giving the body a precursor it can use to synthesize NAD+.


The best option may depend on your goals, medical history, previous experience with NAD+ therapy, preferred route of administration, and how you tolerate treatment.


Some clients may prefer traditional NAD+ therapy, while others may be interested in Niagen because of the research surrounding nicotinamide riboside and NAD+ production.


IV Infusion vs. IM Injection

An IV infusion allows the treatment to be administered intravenously over a period of time and allows the infusion rate to be adjusted for comfort.


An IM injection is administered into the muscle and can be a convenient option for clients who prefer a quicker visit without sitting through a longer infusion.


The appropriate option—and whether NAD+ or Niagen is suitable at all—should be determined based on your individual health history, goals, and clinical assessment.


The Bottom Line

NAD+ is one of the most interesting areas in modern cellular and longevity research, but it’s also an area where marketing can sometimes move faster than science.


Here’s what we know: NAD+ is essential for cellular energy and metabolism. Nicotinamide riboside, including Niagen, can increase NAD+ availability in humans.
